Before you use this form, please check the call number of the title you wish to request. This form should ONLY be used for items that display the call number "In-Process" or the message "Under Consideration" in the Library Catalog.

If the item has a call number, please use the Request option from the catalog record.

If the item is "On-Order" in the Library Catalog, use Request an On-Order Item.

If the item is not listed in the Library Catalog, you can request that the Libraries purchase it using Recommend a Book Purchase.

To submit a request please supply all requested data, then choose Submit Request. To clear the form choose Clear Form.
